Qualitative Respiratory Mask Fit Test

Please complete the registration form below to sign up for a qualitative mask fit test appointment. These appointments take approximately 15 to 20 minutes to complete. Certification is provided immediately upon completion of the appointment. Transfers within 72 hours of the course will be charged a 25 dollar rescheduling fee. Refunds and transfers prior to 72 hours of the course are free of charge.
